Incident Investigation

When something goes wrong, you need answers fast, for your team, your insurer, and your regulator. Our professionals lead or support root-cause investigations, build corrective action plans that hold up to scrutiny, and leave your team with documentation that protects the business going forward.

We work the incident from first notification through final report: witness interviews, scene and equipment review, causal factor analysis, and a corrective action plan your team can actually implement. Not a binder that gets filed away.

Who it's for

Any company that's had a recordable incident, near miss, or Serious Injury and Fatality (SIF) potential event and needs an independent, defensible investigation.

What you get

Investigation report, root cause analysis, corrective action plan, and regulator-ready documentation.

COR Audits & Regulator Readiness

Whether it's a Certificate of Recognition (COR) audit, an internal audit, or a surprise regulator visit, we cover both ends: certified COR auditors in our network can complete your official audit directly, or we can get your program and paperwork ready before someone else finds the gaps.

For readiness work, we start with a gap assessment against the standard you're being measured on, prioritize what actually needs fixing before what's cosmetic, and stay hands-on through remediation, including a mock audit if you want a dry run before the real one.

Who it's for

Companies with a COR audit due, an internal audit or regulator visit on the calendar, or who want to know where they stand before one gets scheduled.

What you get

A completed COR audit by a certified auditor, or a gap assessment, prioritized action list, hands-on remediation support, and an optional mock audit.

Contractor Prequalification Support

Losing a bid because your ISNetworks, ComplyWorks, or Avetta profile fell short has nothing to do with how safe your operation actually is. It has everything to do with how well your documentation answers someone else's checklist. We get your prequalification profile and supporting documentation into shape so it reflects the work you actually do.

For smaller contractors, this is often the fastest path to new client relationships that a full-time safety hire would take months to unlock.

Who it's for

Contractors losing work, or worried about losing work, to prequalification requirements from larger clients or platforms.

What you get

A gap assessment against your target client's requirements, a completed or corrected prequalification submission, and a maintenance plan so it stays current.

Serious Injury and Fatality (SIF) Risk Review

Not all incidents are equal: a scraped knuckle and a fatality don't belong on the same priority list. We help you identify where your real Serious Injury and Fatality risk lives, so your safety effort goes where it actually reduces the risk of someone getting killed or seriously hurt.

That means separating high-energy, low-frequency exposures from the high-frequency, low-severity events that tend to dominate a typical incident log, and building a risk register your leadership team can use to set real priorities.

Who it's for

Operations leaders who want their safety budget and attention pointed at what actually kills and maims people, not just what generates the most paperwork.

What you get

Serious Injury and Fatality (SIF) exposure assessment, prioritized risk register, and practical control recommendations your team can act on.

Critical Control Assurance

Most safety programs can tell you a control exists. Few can tell you whether it's actually being tested the way the work really happens. We review the critical controls tied to your highest-consequence exposures and check whether verification is confirming the control exists, or confirming it works.

This isn't another audit layer. It's a focused look at the handful of controls standing between your operation and a Serious Injury or Fatality (SIF) event, and whether your current verification approach would catch it if one started to fail.

Who it's for

Companies who've already identified their Serious Injury and Fatality (SIF) exposures and want confidence their critical controls will actually hold up in the field, not just on paper.

What you get

A critical control inventory tied to your top exposures, a verification gap assessment, and practical recommendations to close what's missing.

SMS Decluttering

Most safety management systems grow for years without anyone taking anything out. Eventually nobody reads them, nobody follows them, and everybody's exposed. We go through your SMS with you and cut it down to what your team will actually use, and keep using.

The goal isn't a thinner binder for its own sake. It's a system that matches how your operation actually runs, so your in-house team can maintain it without needing an outside consultant back in every year.

Who it's for

Companies whose safety program has become a binder, or a shared drive, nobody opens.

What you get

A leaner, current SMS aligned to your actual operation, with a maintenance plan your team can own.

Interim Safety Leadership

When your safety lead quits, goes on leave, or was never hired in the first place, the gap doesn't wait for a recruiting cycle. We step in as interim or fractional safety leadership: running your program, attending your meetings, and reporting to your leadership team, for exactly as long as the gap lasts.

Unlike a full-time hire, there's no ramp-up cost and no severance when the gap closes. Unlike a generic staffing placement, you get someone matched to your specific industry and risk profile.

Who it's for

Companies with a leadership gap in their safety function, planned or unplanned, that needs to be covered without a rushed permanent hire.

What you get

A dedicated interim safety lead, scoped to the hours or duration you need, with a handover plan for whoever comes next.

Learning Teams

Traditional incident investigations chase root cause and corrective actions from the top down, often missing what the people doing the work already know. Learning Teams flip that: a facilitated, front-line-led conversation that surfaces how work actually happens day to day, where the gap sits between procedure and practice, and what's really driving risk, without turning the conversation into a blame exercise.

We train your leaders and facilitators to run Learning Teams themselves, so this becomes a capability your organization owns, not a one-off workshop everyone forgets about in six months.

Who it's for

Companies whose incident investigations keep landing on "human error" without anything actually changing, or who want their front-line workers to trust the process enough to speak candidly.

What you get

Facilitator training, a guided run-through of your first Learning Team session, and a repeatable framework your team can own going forward.

Request This Service

HOP Training (Human & Organizational Performance)

Human and Organizational Performance training brings your leaders and supervisors up to speed on why people make the decisions they do in the moment, and why blaming individuals for system failures rarely prevents the next one. It's built around five core principles: error is normal, blame fixes nothing, context drives behavior, learning and improving is vital, and how leaders respond matters.

This isn't theory for its own sake. We tie every session back to your operation's actual incidents, near misses, and everyday friction points, so your team leaves with a different lens for reading their own workplace, not just a new vocabulary.

Who it's for

Leadership teams and supervisors who want to move past a blame-based safety culture and start addressing the systems and conditions that actually drive outcomes.

What you get

A scoped HOP training session for your leadership team, examples drawn from your own operation, and practical tools to carry the principles into daily supervision.

ERP Development (Emergency Response Plans)

A lot of Emergency Response Plans exist because a regulator or client required one, not because anyone expects them to actually work in a real event. We build site-specific ERPs from the ground up: evacuation routes and muster points, incident command roles, communication protocols, and coordination with local fire, EMS, and industry mutual-aid partners, matched to your actual site, not a generic template with your logo on it.

We also review and update ERPs that already exist but haven't kept pace with site changes, staffing turnover, or new hazards, so the plan on file still matches the plant, camp, or job site it's supposed to cover.

Who it's for

Companies that need a new ERP for a regulator, client, or insurer requirement, or whose existing plan hasn't been touched since it was first written.

What you get

A complete, site-specific Emergency Response Plan, incident command structure, and a maintenance schedule so it stays current.

Request This Service

Emergency Response Support

A plan that's never been tested is a guess. We run tabletop exercises and full-scale drills against your ERP, train your emergency response team and incident commanders, and identify the gaps between what the binder says and what your people would actually do at 2am when something goes wrong.

We can also provide on-call incident command support during an actual event: an experienced outside responder on the phone or on site to help your team manage the first critical hours, when internal resources are stretched thin and the stakes are highest.

Who it's for

Companies with an ERP already in place who need to test it, train their people against it, or want experienced backup during an actual emergency.

What you get

Drill or tabletop facilitation, a gap report against your existing plan, emergency response team training, and optional on-call incident command support.

Custom / Ongoing Advisory

Not everything fits into a clean category. If you've got an H&S problem that doesn't have a name yet: a merger bringing two safety programs together, a client demanding something your program doesn't cover, a leadership team that wants an outside read on their risk exposure. Tell us what it is.

We'll scope it, quote it, and bring in the right professional. If it turns into something ongoing, we can structure that too, still on your terms, still without a long-term contract if you don't want one.

Who it's for

Anyone with an H&S need that doesn't fit neatly into the categories above.

What you get

A scoped plan and quote within one business day of describing the problem.
